Where can users locate / see the file index and folder hierarchy when using WP File Download?

Can uses see (and download) an index of the folder structure and files?

Users can see your folder structure if you set the visibility to be public on all your folders.
It is not possible to download the structure in our plugin at presently.

Can we have the root folder require a username and password and have all the subfolders be visible on you've logged into the root (top level) folder?

Also, is there an example installation where I can see the client / customer facing side interface?

It could be said that, the page including folders and files will be blank for public. A user can see the root folder and its sub once he logged in.
You should customize a login form in the page if needed.
