Is it possible to put a "form lock" on a file so that users are required to fill out a form before they can download a file (similar to the social lock add-on but requiring the user to fill out custom form fields instead of social media)?

I'm sorry, this feature is not implemented yet. You can only lock the files by Social locker now.
