Adding a Document Entry without Adding a PDF Download

We should be able to add an entry with Title and Description, even if we don't have the .pdf for it.

For example if I want to make a full Reading List and only add the pdfs later (whenever they are scanned).

That would be great! That way the plugin can serve as a list plugin at the same time.
