File Download Insert Image

If you mean add an image in your content and display it, WP Media Folder is more for you (it's an enhancement of the WordPress Media Manager).
WP File Download is a file manager, you can "only" add downloadable files. The file management permissions can be managed in WP File Download.

I could use file download to manage user access and media manager for users to insert images if I could have media manager and file download point to the same location. What would happen if media manager synced with itself?

This is for an internal website, so I could control access to folders using IIS user control (similar to restricting their FTP access to only specific folder in Uploads), then remove their ability to upload anything new, and have media manager sync what is in the uploads folder into the media library. Can I set media manager to auto sync new files every time something is uploaded into the uploads folder via ftp? Or have users be able to sync their own files. It would be a bit annoying for them, but if they could just click a sync button every time they add a file, it should work. However, I would need sync to remove files from the media library if they are not on the server.

Currently, in WP File Download plugin you can import files from FTP manually; however, the sync server folder feature is something we have planned for a future major release.

In WP Media Folder plugin, the feature is already in, but it cannot be synchronized from our plugin to the server.
