Is it possible for individual users to upload documents and assign them to their folders?
For example:
On Monday user A upload files to folder A
On Monday user B upload files to folder B
On Tuesday user A upload files to folder A
etc.
Or will all documents be collected into one big folder?
For example:
On Monday user A upload files to folder A
On Monday user B upload files to folder B
On Tuesday user A upload files to folder A
etc.

Users can see the form and upload to that category on frontend if they have access to that folder.
You can choose a category when creating a shortcode of upload form. For more information:

Yes, but on your post/page, you should create 3 upload forms (shortcode) for 3 users; for example.
Then you should insert 3 shortcodes on your post/page. Finally, on frontend it will be :
User A can see the upload form of Category A
User B can see the upload form of Category B
User C can see the upload form of Category C
I know this is so manually; however, this is only way at the moment.

This is what I am looking for with one question. I want a user to login and be I redirected to their page with short code. So it is a secure page where they can upload their images. Is this possible?
