Hi, will the folders created in the Media Library be reflected in the actual upload folder?

Yes as mentioned it uses the WordPress native media categories. Technically we've chosen this way for the following reason:
- This is really fast to manage for final user
- Once you uninstall/re install the plugin you won't loose any content.

If there was an option where folders are created, so I can actually see the new folders when I log on via FTP, then this would be a perfect plugin for me.

You "can't" create real folders in WordPress and keep the default media manager compatibility.
Well you can but when you'll uninstall the plugin everything will be lost. It's too much implanted in the core of WP.
Or you can use Joomla for that

#1783 Yes that's true, we have take a look at a similar solution but in this case the plugin is using your own interface/button in editor to call the media manager. Well, you can do almost everything but then the problem become is it easy to maintain? what is the best compromise?

#1803 I appreciate the fact that this plugin doesn't try to do too much making it safer faster and easier to maintain.
If there is a solid demand for actual folders maybe a separate "export images to categorized folders extension" would be cool.

Yes!! That would make this an exceptionally useful plugin.
I am in the process of migrating a site with 500+ images in 30+ galleries. Fortunately the original site used NextGen to organize the image galleries - folders. It was easy for me to upload the images into new native Wordpress (per page) galleries. I am already concerned about the next migration.
Having the ability to export the Wordpress gallery images (or other taxonomy) would be huge. This could be a single zipped archive of the gallery images, page attachments, or a WP Media Folder.

Let's distinguish between WP Media Folder's virtual folders (VF) and filesystem folders (FSF).
My wish list would include:
Export VF and contents as archive
Import FSF to VF
Move media from one VF to another VF
Attach/Unattach VF to post/page
Auto create VF based on taxonomy (post/page attachment, author, date/time, other)
Sequential rename of VF contents. (just having fun with this)
Import FSF to VF and Attach/Unattach VF to post/page would easily take care of the need for converting/importing NextGen galleries. Besides, there are already other methods of converting NextGen Galleries to WP Galleries.

I have been looking for an ideal solution for displaying my image libraries (already arranged into a logical folder structure using WP Media Folder) as a set of galleries. I came across Envira Gallery, but this plugin cannot interpret WP Media Folder's VFs, and there is seemingly no way to export the folder structure that I have worked hard and invested a lot of time to organise into any kind of format that would be compatible with Envira.
It is a nice idea that we can uninstall WP Media Folder and not lose our media, but not being able to preserve the structure in some kind of exportable format in addition to the content means that we are losing a great deal of our work.
It would be a relatively easy task to simply perform a bulk upload of all our content again, it is the organising and structuring of that content that would be frustrating and time-consuming to recreate.

We haven't planned any Envira gallery export tool, I would even say that it's up to Envira team to do such integration (as it's up to us to create an Envira importer).
Currently we are finishing an addon that adds an integration of Google Drive, Dropbox and a PDF embed feature in the media manager. It should be finished this week. Right after the cloud integration we will work on a full gallery addon for WP Media Folder. And of course it'll have an integration with our folder management
